Weekend Rotation:
Every 6th Saturday and Sunday working 8am to 2pm
PM Shift Rotation: Every 6th week working Monday through Friday PM shift of 1pm to 9pm
Holiday rotation is also required.
Position Purpose:
The Coder/ Transcriptionist is a medical language specialist who transcribes dictation by physicians and other healthcare professionals to document patient care and facilitate delivery of health care services.
The Coder/! Transcriptionist is also responsible for coding and abstracti! ng inpatient as well as ambulatory surgery and observation records.
Also, the Coder/ Transcriptionist assists in any other areas including ER/Urgent Care, outpatient and abstracting and other documentation-related processes.
MAJOR JOB FUNCTION:
1.
Responsible for coding all inpatient records, all ambulatory surgery records, and all specialty clinic records.
Acceptable standards are: Inpatient coding â" 3 charts/hour, Day Surgery, Ambulatory Infusion and Blood Transfusion â" 4 charts/hour, Emergency Room â" 11 charts/hour, Urgent Care â" 15 charts/hour, Outreach Clinics â" 20 charts/hour and Outpatient Ancillary â" 40 reports/hour.

Responsible for assisting, if necessary, Outpatient Coding Specialist.
3.
Responsible for abstracting, keying and transmitting of both inpatient and outpatient diagnoses.
4.
Responsible to perform the following activities on a sched! uled basis (to maintain efficiency) as well as back-up in absence of other staff:
Coding of outpatient records â" E/M, ICD-9-CM and CPT â" in absence of Coding Specialist.
Discharge analysis of inpatient, observations and same day surgery records.
Outpatient record process in absence of Outpatient Coding Specialist.
Scanning and quality checking of medical record reports.
Assists HIS Director in certain quality assurance activities (including Surgical Case Review, Blood Usage Review), and provides clerical and other support as requested in other quality assurance activities.
Responsible for obtaining signatures for completion of medical records for ER physicians.
Responsible for taking medical records of discharged patients to attending physicians on a daily basis in order to obtain final diagnoses and procedures to facilitate timely billing of diagnostic information.
Responsible for notifying Adminis! trator of charts that do not have a doctorâs summary completed within! 15 days..
Releases health care information to insurance companies, Medicare, other health care facilities and physicians, attorneys, etc., in accordance with established policies and procedures.
Copies all records for Work Comp billing and maintains HMS log of requests for information and response to requests for healthcare information within three working days as much as possible.
Applies knowledge of current legislation affecting confidentiality and release of information and utilizes reference manual.
5.
Completes reporting in a timely manner according to the state of Wisconsin regulations to include: cancer reporting and reporting to peridata.
6.
Transcription duties:
Logs dictated reports and retrieves final diagnosis
Transcribes dictation from physicians to include histories, physicals, operative reports, discharge summaries, consultations, delivery notes, progress notes, ER records, S! pecialty Clinic records and any special correspondence for physicians regarding patients.
Must meet timeliness standards.
Transcribes all QA dictation.
7.
Responsible for merging duplicate medical record numbers.
8.
Orients new staff to the department as assigned.
9.
Participates in staff meetings, appropriate in-services and competency testing.
10.
Responds to changing priorities of the workload in a flexible manner.
11.
Operates and performs necessary maintenance on equipment.
Reports any problems or malfunctions to the service center and supervisor promptly.
12.
Answers the telephone, takes massages and communicates the messages to the appropriate person.
13.
Performs all other duties as assigned.
Qualifications/Skills:

High school diploma or equivalent.
·
Associates Deg! ree in Health Information Technology, completion of formal courses in me! dical secretarial science or equivalent experience.
·
RHIT or CCS Certification preferred.
·
Extensive background in medical terminology, basic anatomy and physiology, surgical terminology and pharmacology.
·
Training and/or experience in ICD-9-CM coding and E/M coding.
·
Excellent verbal and written communication skills.
·
Excellent human relation skills as demonstrated by the ability to interface positively with all employees.
·
Must have the ability to work with frequent interruptions, under stress and deadlines, with minimal supervision, and to exercise initiative and judgment in analyzing, organizing, planning, prioritizing, scheduling and coordinating work with others.
·
Personal computer and word processing experience required.
Microsoft Office experience preferred.
·
Manual dexterity required for computer us! e.
Also needs the ability to stay at one workstation for long periods of time.
