Summary: The VAD/Heart Transplant Coordinat! or will be responsible for the coordination of care for all patients requiring implantation of a cardiac assist device. These include patients accepted for, and awaiting heart transplant, or those with Advanced Heart Failure in which the device is implanted as a permanent treatment option. The VAD/Heart Transplant Coordinator will also be responsible for the coordination of care for all patients who are pre- or post- heart transplant. The VAD/Heart Transplant Coordinator will work as a member of the Advanced Heart Failure Team, and will be the primary liaison with the transplant cardiologists and cardiac transplant surgeons, other physicians, and health care staff throughout the hospital. A major component of the position is patient and family education, daily rounds, discharge planning, and on-call coverage. The VAD/Heart Transplant Coordinator is also responsible for outreach activity to promote the programs, frequent communication with referring physicians, and professio! nal education. The VAD/Heart Transplant Coordinator functions ! within the parameters outlined in the LVAD practice guidelines.
Experience: Minimum of 3 years nursing experience in a critical care/acute care setting is required.
Licensure: Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within three months of hire. American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) is required within one year of hire.
Education: Bachelor's degree in Nursing is required.
