Advanced Practice Nurse

Provides primary or specialty health care services within defined scope of practice in collaboration with! a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures.

Department Position Purpose

Provide professional healthcare to patients in collaboration with a licensed physician and in accordance with policies and procedures. Works across multiple settings including inpatient/outpatient clinic. May need to participate in off shift coverage.

CORE RESPONSIBILITIES

1. Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.

2. Write Orders and interprets laboratory and diagnostic tests with appropriate communication and documentation

3. Develops pharmaceutical and non-pharmaceutical treatment plans.

4. Monitors effectiveness of plan of care and modifies as necessary under the supervision of a physician.

5. Educates patients on disease and treatment plans.

6. Performs procedures as traine! d and appropriately credentialed.

7. Documents all fi! ndings within the confidential Electronic Medical Record (EMR) and completes other necessary documents.

8. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors including preoperative and postoperative care.

Department Specific Responsibilities

9. Works collaboratively within a multidisciplinary team to provide care to CT surgical patients in both an inpatient and outpatient setting.

10. Provides follow up/post-op care in the inpatient and outpatient (clinic) settings.

11. Attends daily rounds with the Attending and physician team, presents 24 hour patient report.

12. Performs the following procedures consistent with privileges under the supervision of a physician including but not limited to: 1)placing arterial lines, central venous lines, Swan Ganz catheters and chest tubes; 2) endotracheal tube insertion 3) insertion of intra-aor! tic balloon pump

13. May provide services at multiple hospitals based on the location of clinical practice

14. Writes admission, discharge, transfer, medication and other orders as required for the management of patient care as directed by a physician.

15. Participates in quality assurance activities and/or clinical trials.

16. Participates in activities that contribute to the education of healthcare professionals including (one on one education, and formal presentations).

17. Develops quality relationships with a diverse referring physician network for tertiary level cardiothoracic patients.

18. May involve program development activities to establish new CT surgery programs.

Qualifications

MINIMUM POSITION QUALIFICATIONS

Education Requirements

Advanced Practice Nurse

Master’s Degree in Nursing; APNP Certification as Nurse Practitioner

Experience

  • N! o experience required.
  • Prefer 3 years of experience in cardiac! nursing; with additional experience as a CT surgery NP preferred.
Certifications/Licenses

(Must be current and maintained)
